i dont think ill worry about this one...apparently you still have to buy addons etc to open up options in the game...
Anyone know if this is instead of a monthly fee or in addition to?

no monthly fee, just pay for any chapters they add (expansions).
ie. like EQ has done, or UO, or many of the other countless MMO's.
expansions you pay for, but regular updates/new content with your existing chapter is still free, and streamed to you ingame.
edit: there's an interview with the producer here that goes into more detail on how that all works.
