I've got 2 mice. One Bluetooth Dell mouse that came with my computer, and one Logitech G7 Wireless USB mouse. They both have the same issue... Which means its software.
Basically, sometimes they decide they don't want to click on things... ie, ill put the mouse cursor on a window, and click it, which normally would select that window, and bring it to the front. Funny thing is, it DE SELECTS the window (ie: when you already have a window selected and you click on the desktop) even when the mouse is clearly on the window. It seems to have semi-healed itself in the last few days, but i still have a problem with it once or twice a day. Now, to fix the problem, all i really have to do is press CTRL+ALT+DEL and then close that window down, and it goes back to normal. Sometimes it also decides your right mouse button and left mouse button are reversed (thats a quite rare one though) where you left click and it brings up the right click menu.... to fix that one I just need to right click on something and the computer goes -WTF-That's-a-right-mouse-click-so-the-other-one-must-be-a-left-click and it works fine again. (that one happened just then) The last issue is that sometimes it decides it won't click on anything... you click on something and it ignores you. Some things will click, some things won't. I think that is actually related closely with the first issue I described.
I've tried a destructive Windows XP MCE rebuild, and the problem re-asserted itself quickly thereafter.
I've looked in device manager where its got both mice and two HID COMPATIBLE MOUSE. i've disabled the those two HID COMPATIBLE MOUSE devices in there, and I think that has been how it has "semi-healed" itself. I've checked for newer drivers both by DELL and Logitech, yet the issue still occurs...

answer 1: format... or apply extreme forward velocity of hdd into nearest solid object.
answer 2: delete the stock standard windows mouse drivers and re-install.. be sure you have a copy of the mouse drivers or a windows cd around.
for answer 1, I have already wiped windows off the HDD and reinstalled it from scratch... lost a few good programs that day,but found 60GB of hard disk space somewhere...
for answer 2, the logitech mouse uses the setpoint drivers... and i'm having issues with that too. i've tried deleting them from device manager and re-detecting them, but the issues still occur
